This presentation focuses on integrating engagement throughout the workshop by facilitating small group breakouts for hands-on practice in creating a community of practice and planning technology use across the spectrum of recovery. Case examples will be used for interactive problem-solving and discussion, incorporating the knowledge-to-action cycle. The session will demonstrate how technology usage evolves throughout the continuum, with varying goals depending on the setting, and will incorporate polling to generate summaries for discussion.

Using Sheltering Arms Institute as a case study, we will discuss how evidence-based practice and patient-centered care shaped its creation, including input from leadership, stakeholders, and the rehabilitation team. The Clinical Science Department was established to bridge the 17-year gap between research and clinical practice, with a focus on technology adoption, training, and implementation across the continuum of care. We will explore methods for measuring progress and outcomes, including clinical outcome collection, electronic medical record data on technology usage, and dashboarding for visualization by diagnosis, clinic, and clinician. The clinical science team is also working with statisticians to analyze the relationship between technology usage and patient outcomes, with individualized clinician feedback.

This two-part workshop will cover the integration of inpatient and outpatient rehabilitation care, leveraging successes and overcoming barriers related to technology and intensity to maximize patient outcomes. We will take attendees through the process of navigating a culture of change at SAI—from initial research and operational planning to training, implementation, and outcome assessment. Additionally, we will discuss the benefits of creating a community of practice to help clinicians increase knowledge and optimize outcomes through shared training and translation resources.
